Hunter Gatherer are supporting a great Remote working role and in search for a Band 6 Health Visitor. To do this they have a large workforce of the best healthcare individuals.

The post holder will be a nurse with the appropriate professional or regulatory body (NMC).They will have clinical experience working with adults in a community setting with emotional wellbeing and mental difficulties, they will have significant experience of delivering clinically robust mental health assessments, care planning and risk. 

Job Details:

• Start Date: ASAP
• End Date: Initially 3 months, ongoing contract

• Shift Pattern: Monday - Friday 9-5

• Specific Skills:
• Compressions
• Diabetes management (insulin etc)
• Standard community nursing duties

Key details of the role:

• To review a vacant case load of level 2/3 families and make decisions on plan of action following triage.

• Client to be booked into a clinic for follow up, book child health review as needed or liaise with team lead for allocation and do a telephone assessment for follow up, and discharge accordingly.

• May be required to cover case conferences / strategy meeting if team member unavailable or as per safeguarding team support required.
• Start date – ASAP
• Duration – 3 month- 6months ongoing contract
• Full Time position
• £35.00 PH
