Band 5 Community Nurse

Hunter MH are currently searching for experienced, dedicated and committed Band 5 Community Registered Nurse on a full-time basis (37.5 Hours, Monday – Friday 8:30 – 5)

We work with an outstanding range of Public and private healthcare settings within Cheltenham and Cirencester.

You will work as a part of a multidisciplinary team, providing nursing care to patients suffering from a wide variety of medical conditions and illnesses, supporting in their treatment, recovery, and well-being.

 Requirements

• NMC registered nurse
• Current resident and Right to work in the UK!
• Highly experienced Community Band 5 nurse 
• Competent using System 1 software.
• Assess patient and carers needs through strength based assessment.
• Develop individual care plans in partnership with patients and carers. This will include information, advice, sign-posting and education.
• Regularly evaluate care plans to ensure they are up to date and appropriate, reporting clinical changes or areas of concern to the appropriate member of the team.
• Provide skilled nursing care interventions, support and advice to colleagues, patients and carers.
• Work to reduce unwarranted variation in clinical practice, providing evidence based intervention

Must have:

• Current valid NMC registration
• Current satisfactory Enhanced DBS Certificate#
• Experience working as a DN within a community setting.
• You must have the relevant UK experience.
