Duration: 3-6 Months Contract
Hours: Monday to Friday, 30 hours per week

Job Description:
The Out of Area Flow Practitioner position is a pivotal role within the Out of Area Flow Team, based in Preston. As an Out of Area Flow Practitioner, you will play a crucial role in overseeing the care of residents who are placed in out-of-area beds. This involves close collaboration with various stakeholders, including Independent sector Mental Health Acute and Psychiatric Intensive Care Unit (PICU) bed providers, the Bed Hub, and OAPs providers.
Your primary responsibility will be to provide clinical oversight of patients in out-of-area beds, ensuring their well-being and facilitating their safe and timely discharge. This includes monitoring their progress, coordinating with the appropriate healthcare professionals, and advocating for the best interests of the patients.
Key duties and responsibilities of the role include:

Clinical Oversight: Conducting regular assessments and evaluations of patients placed in out-of-area beds to ensure their clinical needs are being met. This involves reviewing medical records, liaising with treating clinicians, and identifying any gaps or concerns in their care.

Collaborative Working: Working closely with Independent sector Mental Health Acute and PICU bed providers, the Bed Hub, and OAPs providers to coordinate the care of patients and facilitate seamless transitions between care settings. This requires effective communication skills and the ability to build strong working relationships with multidisciplinary teams.

Patient Advocacy: Acting as an advocate for patients to ensure their voices are heard and their preferences are considered in decision-making processes related to their care. This may involve facilitating discussions between patients, families, and healthcare providers, and advocating for resources or services to meet their needs.

Discharge Planning: Supporting the safe and timely discharge of patients from out-of-area beds by coordinating discharge planning meetings, arranging necessary services or resources in the community, and providing education and support to patients and their families/carers.

Documentation and Reporting: Maintaining accurate and up-to-date records of patient assessments, care plans, and interventions in accordance with organizational policies and regulatory requirements. Providing regular reports to management and other relevant stakeholders on key performance indicators, trends, and outcomes related to out-of-area placements.
Qualifications and Experience:
- Registered Nurse with current registration with the relevant regulatory body (e.g., NMC).
- Significant experience working in mental health services, preferably with experience in acute or crisis care settings.
- Knowledge of relevant legislation, policies, and best practice guidelines related to mental health care and out-of-area placements.
- Strong organizational and problem-solving abilities, with the capacity to manage multiple priorities effectively.
- Commitment to continuous professional development and maintaining high standards of clinical practice.
