Job description

We are seeking a highly skilled and compassionate Band 6 Community Psychiatric Nurse (CPN) to join our agency team. As a CPN, you will play a pivotal role in providing high-quality mental health care and support to individuals within the community. Key Responsibilities:

  1. Conduct comprehensive assessments of individuals experiencing mental health difficulties, including risk assessments, to formulate personalized care plans.
  2. Provide evidence-based interventions, therapeutic support, and medication management to individuals with varying mental health conditions, ensuring adherence to best practice guidelines.
  3. Deliver psychoeducation to individuals and their families/carers to promote understanding and self-management of mental health conditions.
  4. Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and occupational therapists, to coordinate care and facilitate seamless transitions between services.
  5. Engage in crisis intervention and de-escalation techniques to support individuals in moments of acute distress or psychiatric crisis.
  6. Monitor and evaluate the effectiveness of interventions and care plans, adjusting strategies as necessary to optimize outcomes.
  7. Advocate for the rights and needs of individuals with mental health difficulties, promoting empowerment and autonomy in decision-making.
  8. Maintain accurate and up-to-date documentation of assessments, interventions, and communications in accordance with professional and organizational standards.
  9. Participate in continuous professional development activities to enhance clinical knowledge and skills, staying abreast of advancements in mental health care.
  10. Demonstrate a commitment to maintaining confidentiality, ethical practice, and professional boundaries in all interactions with individuals and colleagues.

Requirements:

  1. Registered Nurse qualification with current registration with the appropriate regulatory body (e.g., NMC in the UK).
  2. Previous experience working as a Community Psychiatric Nurse or in a similar mental health setting, preferably at Band 6 level.
  3. Demonstrated competence in conducting comprehensive mental health assessments, risk assessments, and care planning.
  4. Proficiency in delivering evidence-based therapeutic interventions and medication management within a community setting.
  5. Excellent communication and interpersonal skills, with the ability to build rapport and establish therapeutic relationships with individuals from diverse backgrounds.
  6. Strong organizational and time management skills, with the ability to prioritize competing demands and work effectively under pressure.
  7. Commitment to collaborative working within multidisciplinary teams and a multidisciplinary approach to mental health care.
  8. Knowledge of relevant legislation, policies, and best practice guidelines governing mental health care.
  9. Flexibility to work variable shifts, including evenings, weekends, and bank holidays, to meet the needs of the service.
  10. Valid driver's license and access to a vehicle for community-based visits (where applicable).
