We are seeking a highly skilled and compassionate Band 6 Community Psychiatric Nurse (CPN) to join our agency team. Key Responsibilities:
- Conduct comprehensive assessments of individuals experiencing mental health difficulties, including risk assessments, to formulate personalized care plans.
- Provide evidence-based interventions, therapeutic support, and medication management to individuals with varying mental health conditions, ensuring adherence to best practice guidelines.
- Deliver psychoeducation to individuals and their families/carers to promote understanding and self-management of mental health conditions.
- Collaborate with multidisciplinary teams, including psychiatrists, psychologists, social workers, and occupational therapists, to coordinate care and facilitate seamless transitions between services.
- Engage in crisis intervention and de-escalation techniques to support individuals in moments of acute distress or psychiatric crisis.

- Advocate for the rights and needs of individuals with mental health difficulties, promoting empowerment and autonomy in decision-making.
- Maintain accurate and up-to-date documentation of assessments, interventions, and communications in accordance with professional and organizational standards.
- Participate in continuous professional development activities to enhance clinical knowledge and skills, staying abreast of advancements in mental health care.
- Demonstrate a commitment to maintaining confidentiality, ethical practice, and professional boundaries in all interactions with individuals and colleagues.
Requirements:
- Registered Nurse qualification with current registration with the appropriate regulatory body (e.g., NMC in the UK).
- Previous experience working as a Community Psychiatric Nurse or in a similar mental health setting, preferably at Band 6 level.
- Demonstrated competence in conducting comprehensive mental health assessments, risk assessments, and care planning.
- Proficiency in delivering evidence-based therapeutic interventions and medication management within a community setting.
- Excellent communication and interpersonal skills, with the ability to build rapport and establish therapeutic relationships with individuals from diverse backgrounds.
- Strong organizational and time management skills, with the ability to prioritize competing demands and work effectively under pressure.
- Commitment to collaborative working within multidisciplinary teams and a multidisciplinary approach to mental health care.
- Knowledge of relevant legislation, policies, and best practice guidelines governing mental health care.
- Flexibility to work variable shifts, including evenings, weekends, and bank holidays, to meet the needs of the service.
- Valid driver's license and access to a vehicle for community-based visits (where applicable).
