Job Title: Mental Health Practitioner (RMN/SW/OT) - Band 6

Location: Neighbourhood Team, Kidderminster

Hunter Mental Health is currently seeking a dedicated and experienced Mental Health Practitioner (RMN/SW/OT) to join an esteemed Neighbourhood Team in Kidderminster. This is a locum role with the opportunity to make a real impact in the lives of individuals within the community.

Role Overview: As a Mental Health Practitioner, you will play a crucial role in providing high-quality mental health support and interventions within the community setting. Collaborating closely with multi-disciplinary teams, you will assess, plan, implement, and evaluate care plans tailored to meet the unique needs of service users. This role offers an excellent opportunity to utilize your expertise and contribute to the enhancement of mental health services.

Key Responsibilities:

  • Conduct comprehensive assessments of service users' mental health needs.
  • Develop and implement individualized care plans in collaboration with service users, their families, and multi-disciplinary teams.
  • Provide therapeutic interventions and support aimed at promoting recovery and enhancing well-being.
  • Monitor and evaluate the effectiveness of care plans, making necessary adjustments as required.
  • Work collaboratively with other healthcare professionals and external agencies to ensure holistic and coordinated care delivery.
  • Maintain accurate and up-to-date documentation of all interventions and interactions with service users.

Requirements:

  • Qualified Mental Health Practitioner registered with relevant professional body (RMN/SW/OT).
  • Previous experience working within a mental health setting, preferably in a community or neighborhood team.
  • Demonstrated ability to conduct comprehensive mental health assessments and develop appropriate care plans.
  • Strong communication and interpersonal skills, with the ability to effectively engage with service users, families, and colleagues.
  • Commitment to delivering high-quality, person-centered care in accordance with professional standards and organizational policies.
  • Flexibility and adaptability to respond to changing priorities and work effectively within a dynamic team environment.
