Hunter Mental Health are supporting a great Community Treatment Servcie in their search for a Band 6 Soical worker in a North East London based service The NHS trust is a community and mental health designed trust who prides itself on providing a range of services for all ages in the south east region they cover. To do this they have a large workforce of the best healthcare individuals who work alongside the hospitals and patients homes in the community to continue this passion.

The post holder will be a nurse with the appropriate professional or regulatory body (SWE).They will have clinical experience working with people with emotional wellbeing and mental difficulties, they will have significant experience of delivering clinically robust mental health assessments, care planning and risk. They will have experience of working with people in the community in a mental health service.

The service is open Monday to Friday 09:00 – 17:00. Condensed Hours Available & Weekend Work Available for the right individual.

The role is an office environment working as part of an MDT in an admission avoidance team. Assessing patients over the phone or assisting other members of staff with their assessments. Putting in packages of care, giving specialist advice or arranging 24-Hour Care. Skills reequired would be to assess in a holistic way, triage calls and take leadership of the caseload from an AHP perspective, complete assessments and identifying the need. Respond to care requests. Offer specialist training to other staff members.
