Location: Reading Area (Some remote working possible, to be discussed with management)
Contract: Full-time (Mon-Fri, 9 am to 5 pm) | Long-term vacancy (12 months)
Pay Rate: £25.38 -£27 per hour (Umbrella)
Start Date: ASAP
We are seeking a motivated and experienced Band 6 Community Psychiatric Nurse (CPN) or Social Worker to join our dedicated Adult Community Mental Health Team (CMHT) in the Reading area. This is an exciting opportunity to work with adults aged 18 to 65, providing vital care coordination and support to individuals with varying mental health needs.
About the Role:
As a valued member of the team, you will:
- Caseload Management: Manage a caseload of up to 30 individuals with diverse mental health needs, ensuring tailored care plans are developed and implemented.
- Care Coordination: Deliver holistic care coordination, working closely with service users, families, and multidisciplinary teams to provide person-centered support.
- Diverse Cases: Support individuals with varying severities of mental health conditions, offering interventions that promote recovery and resilience.
- System Expertise: Use CareNotes effectively to document and track care provided.
- Autonomy and Teamwork: Work both independently and collaboratively within a supportive team environment.
Requirements:
- Qualifications: Registered Mental Health Nurse (CPN) or qualified Social Worker with valid professional registration.
- Experience: Proven experience managing caseloads within an Adult CMHT or equivalent setting.
- Skills: Strong communication, organizational, and problem-solving abilities.
- IT Proficiency: Familiarity with CareNotes or similar electronic record systems.
- Driving: Must have access to a car and hold a valid driver’s license.
- Flexibility: Some part-remote working may be possible, subject to discussion with management.
