Hunter Mental Health have an exciting opportunity for a Band 6 MHP (RMN) to join an CMHT based in Oldham. The role will be supporting people aged 16 and over with severe and enduring mental health and social care needs. Providing holistic assessment, interventions, risk assessments and care plans for their caseload. The team is integrated with the local authority, providing a multidisciplinary approach to the care provided. 

Main Duties:

• Assessment of health and social care needs
• Producing, monitoring and evaluating collaborative care plans 
• Risk assessment and management
• Providing evidence based therapeutic interventions
• Providing education advice, emotional and psychological support
• To promote choice in server user involvement in care planning and risk management

• MHP (Registered Mental Health Nurse)
• Band 6
• Start date – ASAP
• Duration - 3 months
• Full time position
• Hours: Monday to Friday 9am – 5pm
• £25.00ph - £26.00ph via an umbrella
