You will work closely with a multidisciplinary team to provide compassionate, patient-centered care, ensuring that each individual receives the highest standard of treatment and support.

Key Responsibilities:

• Conducting comprehensive assessments and developing tailored care plans.
• Administering medications, treatments, and therapies.
• Monitoring patients' conditions and responding to any changes.
• Educating patients and their families on disease management and self-care.
• Collaborating with healthcare professionals to coordinate and ensure continuity of care.
• Providing emotional support and promoting a positive patient experience.
• Documenting patient records accurately and maintaining confidentiality.

 Requirements:

• Registered Nurse (RN) qualification and valid nursing license.
• Experience in community nursing or a similar role is preferred.

• Flexibility to adapt to changing situations and prioritize effectively.
